Bringing Mobility to Patient Care

AdaptCare is an application designed to dramatically increase patient healthcare outcomes during an emergency. It displays a patient’s adaptive care plan from a hospital’s electronic medical records to a mobile device using a QR code. The QR code is printed on a wristband, sticker or similar item that remains with the patient giving emergency healthcare providers, such as EMTs, firefighters and other personnel who may not be credentialed at the hospital instant access to a patient’s particular needs to ensure continuity of care. AdaptCare can also alert providers that the patient may be non-verbal, deaf, blind, have sensory triggers or other special communication needs, even when a family member or guardian isn’t present.

This gives emergency healthcare providers, such as EMTs, firefighters and other personnel who may not be credentialed at the hospital, instant access to a patient’s particular needs to ensure continuity of care.
